Document changes prompt training automatically.

When a document is approved with a substantive revision, the team members who depend on it are automatically prompted to acknowledge the change in their Training Hub. No separate distribution list, no manual triggering, no missed updates. Substantive vs. minor controls whether training is required; acknowledgement is timestamped and pinned to the exact version.
